Quality Use of Medicines

 

A Home Medicines Review is a way for your pharmacist and GP to work together to help you safely use your medications to their best advantage. If you can say yes to any of the following, you would benefit from an HMR:

  • Have you recently been in hospital?
  • Do you see more than one GP or specialist?
  • Do you take any “over-the-counter” medications or “natural” supplements?
  • Do you take more than 5 different types of medications?
  • Do you have any concerns about your medications, such as side effects or why you take them?
Your GP can refer your pharmacist to visit you at home and discuss your concerns. Many people have been helped with issues such as:
  • Explaining where medications should be stored
  • Helping to remember to take medications regularly
  • Checking that inhalers and other devices are being used correctly
  • Making sure that the medications you take are the best ones for you and your current health conditions.

As a result of such a visit many people say they are more confident in using their medications and have a better understanding of how they can maintain or improve their health.

 
Anyone can have an HMR - there is no age restriction - and it doesn’t cost anything!
