Simply enrol in a "Living Life with a Long Term Conditions" Workshop today! Rockingham Kwinana Division of General Practice is offering this workshop, which has been specially developed by Health Professionals for people who are suffering one or more long term conditions, or for people caring for a family member suffering a long term condition.
The workshop provides general information and teaches the latest practical strategies and skills for coping with problems resulting from your condition.

Why is Living Well with a Long Term Condition Management important?
Long Term Condition's, such as asthma, osteoarthritis, diabetes and heart disease, are estimated to be responsible for around 80% of the burden of disease and injury in Australia.
It is estimated that more people in the Rockingham Kwinana area are reported to have higher rates of Asthma, Diabetes, Cardiovascular Disease and Mental Health Disorders than elsewhere in Western Australia. (PHIDU, Nov 2005.) Also noted in is the higher levels of people who have asthma and are smokers, compared to Perth or Australia.
Responding to this significant burden of disease requires a shift in health care from a reactive system with a focus on acute care to one that is proactive and which supports the management of chronic disease. Self-management support is one of a number of elements essential for improvements in the care of people with chronic condition.
