Lifescripts Plus LIFESCRIPT PLUS REFERRAL CARDS PURPOSE
The purpose of the Lifescript Referral Program is to focus on chronic disease prevention in general practice. This program provides an avenue for General Practioners to refer patients to an exercise facility or a personal trainer.
AIMS / OBJECTIONS
· To encourage better health and healthier lifestyle.
· To maintain a positive healthy relationship with the exercise facilities and personal trainers within Rockingham and Kwinana area.
· To expand the exercise facilities and personal trainers options within Rockingham and Kwinana area.
· To reduce the burden of chronic disease on the current health system
TARGET AUDIENCE
The target audience would be any male or female who has a chronic disease or is at risk of developing a chronic disease.
PROCESS
The patient is reviewed by their GP. The GP gives the patient a referral card with the Division’s phone number. It is the patient responsibility to contact the division, once contact will be made, the Living Well Coordinator refers patient into an appropriate Lifescript Plus Option (exercise facility or personal trainer). A referral form is sent out to the patient with relevant information regarding the facility. A letter will also be sent to the GP once the person has been referred and after they have attended.
